Gen-terapija koja je desetak pacijenata s urođenom gluhoćom vratila u svijet zvukova unutar samo nekoliko tjedana zvuči kao priča iz znanstvene fantastike. No, studija objavljena u Nature Medicine, koju su vodili istraživači s Karolinska Instituteta i kineskih bolnica, donosi konkretne brojeve: kod svih 10 sudionika s mutacijom gena OTOF – odgovornog za proizvodnju otoferlina, proteina ključnog za prijenos zvuka u mozak – zabilježena je poboljšana detekcija zvuka od prosječnih 106 dB na 52 dB nakon šest mjeseci.

Djeca u dobi od 1 do 8 godina pokazala su najizraženiji odgovor, dok su tinejdžeri i mladi odrasli (do 24 godine) imali umjerenija poboljšanja. Terapija se sastoji od jedne injekcije vektora baziranog na AAV (adenovirus-associrani virus) koji u unutrašnje uho isporučuje funkcionalnu kopiju gena OTOF.

Prema riječima Maoli Duan, docentice na Karolinska Institutetu, ovo je „ogroman korak naprijed“ jer cilja uzrok gluhoće – nedostatak otoferlina – umjesto da simptome maskira kao što to čine kohelialni implantati.

Međutim, važno je istaknuti da se radi o fazi istraživanja, a ne o odobrenoj terapiji: studija je obuhvatila samo 10 pacijenata, s praćenjem od 6 do 12 mjeseci. Ono što mediji često propuštaju istaknuti jest da ovo nije univerzalno rješenje za sve oblike gluhoće.

Terapija je usmjerena isključivo na pacijente s mutacijama u genu OTOF – koje, prema procjenama, čine oko 1–8% slučajeva urođene gluhoće. Za usporedbu, najčešća genetska mutacija povezana s gubitkom sluha, GJB2, zahvaća do 50% slučajeva u nekim populacijama, a za njezin tretman još uvijek nema klinički testiranih gen-terapija.

Rezultati su impresivni, ali i preliminarni. Svi pacijenti su nakon terapije pokazali poboljšanje u detekciji zvuka, no studija nije jasno definirala razinu funkcionalnog sluha – npr. mogu li razumjeti govor bez dodatnih pomagala.

Ključno pitanje ostaje koliko će ova terapija koštati i će li biti dostupna izvan specijaliziranih centara – posebno u zemljama kao što je Hrvatska, gdje genetsko testiranje gluhoće još uvijek nije rutinska praksa. Ovo će biti važan faktor u određivanju općeg uspjeha ove terapije u praksi.
