Robotska bronhoskopija više nije samo futuristički pojam iz prezentacija bolnica. Studija Mayo Clinica pokazuje da robotizirani pristup može postići 85% osjetljivost na malignitet i znatno smanjiti komplikacije, što je stvaran klinički pomak, a ne PR na bolničkom webu. Uz to, udio rano otkrivenih slučajeva raste, pa tehnologija očito ne rješava sve, ali rješava nešto važno.

Ipak, brojke iz najbolje opremljenih centara nisu isto što i nacionalni standard. Sve što izgleda uvjerljivo u Mayo klinici postaje puno teže u bolnicama s manjim budžetom, manje obuke i manje prostora za kompleksnu opremu. Tu počinje pravi robotski problem: ne u tijelu pacijenta, nego u bolničkoj logistici.

Drugim riječima, tehnologija radi. Pitanje je tko je može koristiti, koliko često i po kojoj cijeni.

Hardverski trošak i dalje je stvarna kočnica. Shape-sensing sustavi traže preciznu kalibraciju, redovno održavanje i obučene timove, a sve to povećava ukupnu cijenu primjene. Manje bolnice zato neće kupovati samo uređaj, nego i cijeli operativni ekosustav oko njega.

Tu je i pitanje obujma: ako sustav koristiš prerijetko, investicija se teško opravdava. Ako ga koristiš dovoljno često, tek onda možeš govoriti o stvarnoj vrijednosti. Zato robotska bronhoskopija zasad izgleda kao ozbiljan napredak u velikim centrima, ali i kao podsjetnik da napredak nije automatski jednak dostupnosti.

U medicini, kao i u robotici, najvažnije je ono što uređaj radi za pacijenta nakon što se ugase reflektori.
